Home and Community Based Care Programs

(Access & Intervention, Medicaid Aging Waiver, the Alternatives Program)

Access & Intervention (AI) Services:

Our AI services is a short-term case management program arranging services for qualified senior clients to avoid premature institutionalization to nursing homes or assisted livings. These arranged services may include personal care, homemaking, adult day care, chore service, or medical equipment. AI case mangers train their clients (and/or their caregivers) of how to utilize those services, and continue to stay in their home rather than moving to care facilities. To be eligible for the AI Case Management Services the client must be:

  • 60 years of age or older

  • Homebound and/or high risk of replacement to care facilities as AI Case Manager assessment 

  • Residents of Weber-Morgan Counties

There is no income or asset test. Donation from the client is accepted.

Medicaid Aging Waiver Program:

Our Medicaid Aging Waiver program is a long-term case management program arranging services for qualified seniors age 65 or older to avoid premature institutionalization to nursing homes or assisted livings. These arranged services may include personal care, homemaking, adult day care, respite care, chore service, meals, adult companion, non-medical transportation, or medical equipment.

To be eligible for the Medicaid Aging Waiver Case Management Services, the client must:

  • Be 65 years of age or older

  • Meet nursing home level of care as determined by RN and Case Manager assessment 

  • Meet Medicaid financial requirements

  • Residents residing in Weber-Morgan counties

The Alternatives Program (TAP):

Our Alternatives Program (TAP) is a long-term case management program arranging services for qualified individuals to avoid premature institutionalization to nursing homes or assisted livings. These arranged services may include personal care, homemaking, adult day care, respite care, chore service, emergency response system, adult companion, transportation, or medical equipment. This program is for those age 60 or older however, 25% of clients can be age 18-59.

To be eligible for the Alternatives Program, the client must:

  • Be 18 years of age or older

  • Meet homebound criteria due to frailty and/or disability 

  • Income eligibility can be no more than– $1,883 per month (single), $2,555 per month (married) 

  • Assets can be no more than– $6,000 (single), $12,000 (married)

  • Residents residing in Weber-Morgan Counties
